This study aimed to examine the relationship between postoperative hematocrit levels and perioperative outcomes in patients who underwent robotic-assisted pulmonary lobectomy. The analysis included 729 consecutive patients over an 11.8 year period. The patients were divided into an Anemic group (postoperative hematocrit below 36% for women and below 41% for men) and a Non-Anemic group (postoperative hemoglobin above 36% for women and above 41% for men).<br /><br />The results showed that there was no significant difference in age between the two groups, but the Anemic group had a higher proportion of male patients. Anemic patients also had greater incidences of preoperative weight loss. During surgery, patients in the Anemic group experienced greater estimated blood loss and received more IV crystalloids compared to the Non-Anemic group. However, the rates of intraoperative complications did not differ significantly between the two groups.<br /><br />Postoperatively, Anemic patients had higher incidences of prolonged air leak after 5 days, longer duration of chest tube placement, and longer hospital length of stay compared to Non-Anemic patients. Thirty-day mortality rates were not significantly different between the two groups. Overall survival did not differ significantly between the two groups either.<br /><br />In conclusion, this study found that Anemic patients who underwent robotic-assisted pulmonary lobectomy had higher estimated blood loss and worse postoperative outcomes, such as prolonged air leak and longer hospital stay. However, overall survival rates were similar between Anemic and Non-Anemic patients. These findings suggest that postoperative hematocrit levels may be a useful predictive factor for outcomes and should be considered in clinical decision-making.
