Patients with non-small cell lung cancer (NSCLC) and EGFR exon 20 insertion (EGFRex20ins) mutations have unclear treatment preferences when it comes to the benefits, risks, and treatment-related attributes. To address this, a study was conducted to assess the trade-offs patients are willing to make using multi-dimensional thresholding (MDT) to improve patient-centered treatment.<br /><br />167 adults with NSCLC and EGFRex20ins or other EGFR mutations completed an online survey in the US. Participants were recruited through physician referrals or a patient advocacy group. They ranked three treatment options: oral pill (once daily), one intravenous (IV) infusion (twice in the first week, once weekly in the next 3 weeks, and then once biweekly), or 3 IV infusions (three drugs separately on one day: once weekly for the first 4 weeks, then once every 3 weeks).<br /><br />The study identified six key treatment-related attributes: disease control rate (DCR), risks of severe diarrhea, nausea/vomiting, rash, fatigue, and treatment regimen. Participants ranked improving DCR as the most desirable attribute (58%), followed by switching to a preferred regimen (17%), and reducing risks of severe side effects. The majority (89%) preferred a daily oral pill requiring cardiac monitoring over IV infusions.<br /><br />Participants were willing to trade-off a percentage of DCR to switch from IV infusions to a daily oral pill. For a 1% decrease in the risk of severe side effects, participants were willing to forgo a certain percentage of DCR. The importance of DCR varied across subgroups.<br /><br />In conclusion, patients with NSCLC and EGFRex20ins prioritize treatment efficacy but also consider treatment-related attributes. Shared decision-making should include discussions on treatment risks and mode of drug administration. This study provides insights into patient preferences that can inform the development of patient-centered treatment strategies for this specific population.
