EP02.02. Combination of Radiotherapy and PD-L1 Blockade Induces Abscopal Responses in EGFR-mutated Lung Cancer - PDF(Abstract)

This study explores the combination of radiotherapy and PD-L1 blockade as a potential treatment strategy for EGFR-mutated non-small cell lung cancer (NSCLC). While immune checkpoint inhibitors (ICIs) have revolutionized the management of NSCLC, patients with EGFR mutations have lower objective response rates to ICIs. The combination of radiotherapy and ICIs has been shown to control local tumor growth and induce an abscopal effect, which is the destruction of tumors outside the irradiated field. However, it is unclear if this combination can also induce an abscopal response in EGFR-mutated lung cancer. <br /><br />To address this, the researchers collected retrospective data from 43 NSCLC patients with EGFR mutations who were treated with ICIs alone. They found that only 3 patients had a partial response, and the median progression-free survival was 2.03 months. They also conducted in vitro experiments to determine the optimal radiation dose that activates the immune system and related pathways in EGFR-mutated lung cancer. In a mouse model, they treated mice with radiation plus anti-PD-L1 and observed tumor growth and T-cell responses. They confirmed the role of CD8 T cells in the abscopal effect by depleting them in mice bearing EGFR-mutated tumors.<br /><br />The results showed that combination therapy with hypofractionated radiation and anti-PD-L1 enhanced abscopal responses in EGFR-mutated lung cancer by activating CD8 T cells in the mouse model. This suggests that this combination may be a promising treatment strategy for NSCLC patients with EGFR mutations. The study provides insights into the potential use of radiation and immunotherapy in improving treatment outcomes for this specific group of patients.
