EP03.01. Molecular Characterization of Non-small Cell Lung Cancer in People Living with HIV or Solid Organ Transplants - PDF(Abstract)

This study aims to investigate the molecular characteristics of non-small cell lung cancer (NSCLC) in individuals living with HIV or who have received solid organ transplants. The immune system suppression associated with HIV and organ transplantation increases the risk of cancer, including lung cancer, but the molecular changes in these populations are not well understood. Medical records of NSCLC patients from three institutions were reviewed, and tumor samples from 15 HIV-positive individuals and 9 solid organ transplant recipients were analyzed using genomic and immune profiling techniques. The most common genomic alterations in HIV-positive individuals were TP53 mutations, KRAS mutations, CDKN2A mutations, and ATM mutations. In solid organ transplant recipients, TP53 mutations were also the most prevalent, followed by KDM5C, PIK3CA, and various other mutations. The expression of PD-L1, a marker of immunotherapy response, was similar in all groups. However, RNA sequencing analysis revealed lower levels of neutrophils in the tumor microenvironment of HIV-positive individuals compared to controls. The levels of other immune cells and immune signatures were similar between the groups, except for a higher replication stress response defect score in HIV-positive individuals. The results highlight the importance of comprehensive molecular testing in individuals with HIV or solid organ transplants to identify targetable genomic alterations in lung cancer. While markers of immunotherapy response were similar, differences in immune characteristics were observed. Further research is needed to fully understand the molecular profiles of lung cancer in these populations.
