In this study, researchers analyzed the genes associated with primary immunotherapy resistance in lung adenocarcinoma (LUAD) and their influence on immune infiltration and prognosis. They used publicly available data to identify immunotherapy-resistant genes in LUAD samples and performed consensus clustering analysis to determine immunotherapy-resistant subtypes. By using various algorithms and tests, they conducted multidimensional analysis of LUAD transcriptome data. <br /><br />The study identified 12 immunotherapy-resistant associated differential genes (irDEGs) and divided 516 LUAD patients into two subtypes based on their expression levels. Cluster 1, with higher expression of LAG3, showed lower tumor stage, worse prognosis, and higher levels of infiltrating immune cells compared to cluster 2. The TIDE algorithm showed a significantly lower immune response rate in cluster 1. <br /><br />The researchers also identified a key resistant gene, NT5E, which was highly expressed in LUAD and significantly associated with prognosis. NT5E expression correlated positively with CD274 expression and negatively with tumor mutational burden. A pan-cancer analysis further demonstrated that NT5E was related to unfavorable prognosis in various cancer types. <br /><br />Overall, this study provides insights into primary immunotherapy resistance in LUAD by analyzing immune infiltration and prognosis. The researchers suggest that NT5E could be a potential novel prognostic biomarker and a target for cancer immunotherapy.
