A pilot study conducted in Thailand explored the potential of T-cell receptor (TCR) repertoire profiling as a biomarker for predicting the response to immunotherapy treatment in locally advanced non-small cell lung cancer (NSCLC) patients. The study involved 12 patients receiving durvalumab treatment after concurrent chemoradiation therapy. The patients were categorized as responders (complete response or stable disease) or non-responders (progressive disease) based on their treatment outcomes. TCR sequencing was performed longitudinally during the durvalumab treatment, and TCR diversity and clonal to total TCR sequence ratio were determined using the Oncomine TCR Beta-SR DNA Assay. <br /><br />The results of the study showed that responders had higher TCR diversity compared to non-responders during the durvalumab treatment. This was measured by the Evenness score, which demonstrated a significant difference between the two groups. Additionally, responders displayed a trend of increased clonal to total TCR sequence ratio after completing durvalumab treatment. However, this trend was not statistically significant. <br /><br />Based on these findings, the researchers suggest that TCR repertoire profiling could serve as a potential biomarker for predicting the response to durvalumab in NSCLC patients. They propose that measuring TCR diversity and tracking TCR convergence ratio may be useful parameters for predicting the effectiveness of immunotherapy treatment. Further research is needed to validate these findings and establish TCR repertoire profiling as a reliable predictor of immunotherapy response in NSCLC patients.
