EP04.04. Delay in Lung Cancer Diagnosis as a result of being Diagnosed as Pulmonary Tuberculosis in Riau Province - PDF(Abstract)

This abstract presents a study on the delay in the diagnosis of lung cancer in Riau Province, Indonesia, due to misdiagnosis as pulmonary tuberculosis (TB). The prevalence of TB in Indonesia is high, and there is an overlap in the clinical presentation of TB and lung cancer. This misdiagnosis results in a significant delay in the diagnosis and management of lung cancer. <br /><br />The study was conducted in Arifin Achmad General Hospital in Riau Province, and the subjects were 76 histopathologically proven lung cancer patients between August 2022 and January 2023. The diagnosis delay was defined as patients being diagnosed as pulmonary TB and receiving anti-tuberculosis treatment (ATT) for more than one month since the onset of their symptoms. <br /><br />Results showed that 17 out of 76 patients were initially diagnosed as pulmonary TB and received ATT for more than one month. These patients were predominantly male and above the age of 40. The cytology and histopathological biopsy revealed that all cases were in the end stage, with 12 cases of adenocarcinoma and 5 cases of squamous cell carcinoma. Pre-referral sputum Acid Fast Bacilli (AFB) testing was conducted in only 10 cases, all of which had negative results.<br /><br />The study concludes that the high prevalence of TB and the similarity in radiological findings between TB and lung cancer result in a significant number of lung cancer patients being misdiagnosed and treated for TB. Furthermore, due to the perception of lung cancer as having a high fatality rate, clinicians often start ATT without conducting detailed investigations, further delaying the diagnosis and progression of the disease.<br /><br />In summary, this study highlights the need for improved early detection and screening methods for lung cancer in Riau Province to reduce the delays in diagnosis and ensure timely management of the disease.
