EP04.04. Ten-year CTNB Data Indicate Its Usefulness and Safety, Especially Salvage Situation after Bronchoscopy without Diagnosis - PDF(Abstract)

This study examines the usefulness and safety of computed tomography assisted needle biopsy (CTNB) for the diagnosis of pulmonary lesions over the past 10 years. The data includes 183 patients who received CTNB at Yokohama Municipal Citizen's Hospital. Of these patients, 93% were performed by pulmonologists and the remaining by radiologists. The median age of the patients was 74, with a male to female ratio of 131 to 52. The majority of patients had an Eastern Cooperative Oncology Group Performance Status (ECOG PS) of 0 or 1. <br /><br />The study found that 90% of the CTNB procedures resulted in a meaningful histological diagnosis. The most common diagnoses were non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC), followed by thymoma, thymic cancer, malignant mesothelioma, lymphoma, and other thoracic benign cases. <br /><br />Interestingly, 32% of the cases were performed as salvage procedures after bronchoscopy without a diagnosis. The salvage success rate, meaning achieving an appropriate histological diagnosis, was about 90%. The majority of the lesions detected by CTNB were located inside the lungs and about half of them had a diameter of 50mm. <br /><br />The major complication following CTNB was pneumothorax, affecting 7.6% of patients, and drainage tube insertion was needed in three cases. No air embolism or biopsy-related deaths occurred. <br /><br />In conclusion, CTNB performed at the Yokohama Municipal Citizen's Hospital has been effective in achieving a high diagnostic rate with few severe complications. Consideration of CTNB for undiagnosed lesions following bronchoscopy is recommended.
