EP05.01. Predictive Factors for Hemoptysis Following CT-guided PCNB : The Role of Post-procedure CT Findings and Cough after Biopsy - PDF(Slides)

CT-guided transthoracic needle biopsy (TTNB) is a commonly used method for diagnosing lung nodules. However, it can lead to complications such as pain, pneumothorax, and hemoptysis (coughing up blood). Identifying the occurrence of hemoptysis after the procedure is crucial for timely management. <br /><br />A study conducted at Chungnam National University Hospital in South Korea analyzed 448 patients who underwent CT-guided TTNB between November 2018 and March 2020. The presence of cough after the biopsy and the occurrence and amount of hemoptysis were recorded. The size of newly appearing lesions on post-procedure chest CT scans was also measured.<br /><br />The study found that hemoptysis occurred in 137 out of the 448 patients (30.5%). This included 73 patients (16.2%) who had blood-tinged sputum and 60 patients (13.4%) who had self-limiting hemoptysis. Four patients (0.1%) required interventions such as endotracheal intubation or bronchial artery embolization. There was a significant correlation between hemoptysis and cough after the biopsy.<br /><br />The size of newly appearing lesions on post-procedure CT scans also showed a significant difference depending on the presence of hemoptysis. This suggests that the size of these lesions may be a factor in predicting the occurrence of hemoptysis.<br /><br />In conclusion, the study suggests that the presence of cough after biopsy and the size of newly appearing lesions on post-procedure CT scans are factors that can predict hemoptysis. Physicians should closely monitor patients for the development of coughs after the biopsy to predict the occurrence of hemoptysis. Additionally, clinicians should be aware of the possibility of hemoptysis if new lesions around the biopsy site appear on post-procedure CT scans, especially if the lesions are large.
