EP05.01. Propective Study: Usefulness of D-Dimer Determination for Thromboembolic Disease Screening in Patients with Metastatic Lung Cancer. - PDF(Slides)

This prospective study conducted by Dr. Teresa García Manrique at the University Hospital Virgen Macarena in Spain aimed to evaluate the usefulness of D-dimer determination in screening for thromboembolic disease (TE) in patients with metastatic lung cancer. D-dimer is a product of fibrin degradation and elevated levels are often found in venous and arterial TE and neoplasms.<br /><br />The study included a total of 45 patients with stage IV lung cancer, with 22% having small cell lung cancer (SCLC) and 78% having non-small cell lung cancer (NSCLC). The patients underwent clinical history, physical examination, and chest CT scans. D-dimer values were determined at baseline before starting treatment.<br /><br />The study found that the cutoff point for D-dimer values was set at 500 ng/ml, with higher values possibly indicating TE and lower values ruling out this diagnosis. The results showed that 93% of the patients had elevated D-dimer levels, with a range between 222-19877 ng/ml. However, none of the patients presented with TE.<br /><br />In conclusion, the study found that in patients with metastatic lung cancer, both SCLC and NSCLC, D-dimer determination is not a useful tool for screening for TE. This is because 93% of the patients in the study had D-dimer levels above the cutoff point. The average D-dimer value in the study was 2230.09 ng/ml, with a median of 1000 ng/ml.<br /><br />Overall, this study suggests that D-dimer determination may not be helpful in screening for TE in patients with metastatic lung cancer. Further research may be needed to confirm these findings and explore other potential markers for TE in this patient population.
