EP06.04. Challenge to Detect CTC-derived EGFR Mutation of Non-small Cell Lung Carcinoma by Unique Blood-Filtration Device - PDF(Slides)

The study aimed to detect EGFR mutations in lung cancer using a unique blood-filtration device and cytology-based detection platform. Circulating tumor cells (CTCs) were isolated from blood samples obtained from the pulmonary vein of surgical resected lung specimens. The CTCs were transferred to slide glasses and stained for immunocytochemistry and/or Papanicolaou staining. The number of CTCs was counted, and DNA was extracted for EGFR mutation analysis. EGFR mutations in CTCs were compared to mutations in lung tumor tissue samples.<br /><br />A total of 12 blood samples were studied. CTCs were successfully isolated using the blood-filtration device and identified using immunocytochemistry. DNA was extracted from the CTCs, and EGFR mutations were evaluated using digital PCR assay. Surgically resected lung specimens were used as a comparison for EGFR mutation analysis using PCR assay.<br /><br />The results showed that EGFR mutations were detected in both blood samples and tumor tissue samples. There were 6 matched mutation-positive cases and 4 matched mutation-negative cases between CTCs and tissue samples. Two cases had mutations only in the tissue samples and not in the CTCs. The sensitivity of the blood-filtration device in detecting EGFR mutations was 0.75, specificity was 1.00, positive predictive value was 1.00, and negative predictive value was 0.67.<br /><br />Overall, the study demonstrated the potential usefulness of the unique blood-filtration device in investigating EGFR mutation status in lung cancer. However, further experiments are needed to fully understand its performance. The authors declare no competing interests.
