This study analyzed the distant metastasis patterns and baseline clinicopathological features of advanced non-small cell lung cancer (NSCLC) patients with the KRAS G12C mutation. KRAS mutations are common in NSCLC, occurring in 25-30% of cases, but there is limited real-world data on patients with the G12C mutation. The study included newly diagnosed stage IIIC-IV NSCLC patients who underwent NGS testing between September 2022 and March 2023.<br /><br />The analysis included 62 patients, with a median age of 57 years. Of these, 38.7% had the KRAS G12C mutation. Other KRAS mutations observed were G12V, G12D, Q61H, G13C, and G12R. No other concomitant oncogenic driver mutations were observed in the KRAS G12C mutant group.<br /><br />The study found that KRAS G12C mutant patients were more likely to be current or former smokers compared to patients without the mutation. At the time of diagnosis, G12C mutant patients had a higher frequency of at least 3 different metastasis sites compared to non-G12C patients. They also had a higher incidence of brain metastasis and a higher number of brain metastasis compared to non-G12C patients.<br /><br />In conclusion, the study suggests that patients with the KRAS G12C mutation represent a highly heterogeneous population and have a different metastatic pattern compared to non-G12C patients. The findings highlight the need for better characterization of KRAS mutant patients to personalize treatment strategies. Further research with larger sample sizes is warranted.
