This study aimed to investigate the expression pattern of circular RNA hsa_circ_0067705 in malignant pleural effusion (MPE) and evaluate its potential as a diagnostic biomarker. Malignant pleural effusion is a common complication of multiple cancers, particularly lung cancer, and is associated with poor prognosis. The currently used diagnostic method, pleural fluid cytological examination, lacks sensitivity and accuracy. Circular RNAs have been implicated in carcinogenesis and tumor progression, and their expression profile in MPE is not well understood.<br /><br />Pleural fluid samples were collected from 25 patients recently diagnosed with MPE associated with non-small cell lung cancer, as well as from 25 patients diagnosed with non-MPE (10 with tuberculous pleural effusion and 15 with non-TPE) of matching age and sex. The expression level of hsa_circ_0067705 in the pleural fluid was measured using quantitative reverse transcription real-time polymerase chain reaction.<br /><br />The results showed that hsa_circ_0067705 expression was significantly increased in MPE patients compared to non-MPE and tuberculous pleural effusion patients. The expression level of hsa_circ_0067705 discriminated between MPE and non-MPE with a sensitivity of 72% and specificity of 60%. It also distinguished MPE from tuberculous pleural effusion with a sensitivity of 80% and specificity of 60%.<br /><br />Based on these findings, hsa_circ_0067705 is overexpressed in MPE and may be involved in its development. Additionally, the expression level of hsa_circ_0067705 shows potential as a diagnostic biomarker for MPE and for differentiating it from tuberculous pleural effusion. However, further large-scale studies are needed to confirm its diagnostic performance in MPE.
