Malignant pleural effusion (MPE) is a common complication of cancers, particularly lung cancer, that is associated with a poor prognosis. Current diagnostic methods, such as pleural fluid cytological examination, lack sufficient sensitivity and accuracy. Circular RNAs (circRNAs) have recently gained attention for their role in cancer development and progression. This study aimed to investigate the expression pattern of circRNA hsa_circ_0067705 in MPE and evaluate its diagnostic performance in distinguishing MPE from non-MPE, specifically tuberculous pleural effusion (TPE). The study found that the expression of hsa_circ_0067705 was significantly increased in patients with MPE compared to TPE. The diagnostic performance of hsa_circ_0067705 expression in distinguishing MPE from non-MPE was evaluated using receiver operating characteristic (ROC) curves, which showed an area under the curve (AUC) of 0.695 at a cut-off value of 2.22, with a sensitivity of 72% and specificity of 60%. When distinguishing MPE from TPE, the AUC was 0.784 at a cut-off value of 0.52, with a sensitivity of 80% and specificity of 60%. These findings suggest that hsa_circ_0067705 may be involved in the development of MPE and could serve as a potential diagnostic biomarker for distinguishing MPE from non-MPE and TPE. However, further large-scale studies are needed to confirm the diagnostic performance of hsa_circ_0067705 expression in MPE.
