Pdf Summary
This study examines the characteristics of lung cancer surgery cases in elderly patients and evaluates the usefulness of the Japanese National Clinical Database (NCD) risk calculator. The NCD risk calculator is a tool used to predict mortality and morbidity rates after lung cancer surgery. The researchers retrospectively analyzed data from 360 patients who underwent lobectomy or wider range resection for lung cancer between May 2012 and December 2021.

The patients were divided into three groups based on age: group A (35 patients) aged 80 years or older, group B (80 patients) aged 75-79 years, and group C (245 patients) aged 75 years. The primary outcome was postoperative complications, and the secondary outcomes included operative time, blood loss, duration of drainage, and postoperative hospital stay. Postoperative complications were defined as Clavien-Dindo classification II or higher.

The results showed that elderly patients had significantly higher NCD risk calculator rates compared to younger patients. There was also a trend towards increased blood loss with advancing age. The higher risk calculator rates in the elderly indicated a higher incidence of postoperative complications, as well as difficulty in factors such as operative time and blood loss.

The study concludes that the NCD risk calculator is a useful tool for perioperative risk assessment in elderly patients with lung cancer. It provides valuable information for predicting the likelihood of postoperative complications and aids in the decision-making process for lung cancer surgery in this population. The findings highlight the importance of considering age-related factors when planning and performing surgical procedures for elderly patients with lung cancer.
