A study was conducted to examine the characteristics of lung cancer surgery cases in elderly patients and evaluate the usefulness of the Japanese National Clinical Database (NCD) risk calculator. The number of elderly patients undergoing lung cancer surgery in Japan is increasing, and there is a need for a preoperative risk scoring system for this population. In 2017, the NCD risk calculator was developed based on the NCD in Japan. The study retrospectively analyzed data from 360 lung cancer surgery cases and divided them into three groups: Group A (patients aged 80 and over), Group B (patients aged under 80 and over 75), and Group C (patients under 75). The results showed that the NCD risk calculator was useful in predicting mortality, mortality and morbidity, bronchopleural fistula, and respiratory failure in older patients. However, complications such as bleeding, operative time, blood loss, and postoperative hospital stay were more common in the older age groups. The study also found that older patients had lower respiratory function and were more likely to experience adhesions and intraoperative air leakage. Therefore, careful perioperative management is important for surgery in elderly patients. In conclusion, the NCD risk calculator is a valuable tool for assessing perioperative risk in elderly patients undergoing lung cancer surgery. Older patients have higher risk calculator rates, indicating a higher incidence of postoperative complications and greater difficulty in the operation. The study highlights the importance of considering patient age and risk factors in surgical decision-making for elderly lung cancer patients.
