EP07.01. Ablation of Early-Stage Non-Small Cell Lung Cancer in Patients With Interstitial Lung Disease: A Multicenter Retrospective Cohort Study - PDF(Abstract)

This presentation discusses a multicenter retrospective cohort study that looked at the ablation of early-stage non-small cell lung cancer (NSCLC) in patients with interstitial lung disease (ILD). ILD, particularly idiopathic pulmonary fibrosis (IPF), poses challenges for treating NSCLC due to the risk of acute exacerbation of ILD following therapy. The study aimed to describe adverse events (AE) and 90-day mortality following percutaneous ablation of NSCLC in patients with ILD.<br /><br />The study included 33 patients who underwent percutaneous ablation of 43 early-stage NSCLC tumors. 51% of patients had imaging findings compatible with IPF. The most common ablation methods used were cryoablation (50%), radiofrequency ablation (26%), and microwave ablation (24%). The primary outcome was the occurrence of any adverse events within 90 days post ablation. The secondary outcome was hospital length of stay.<br /><br />The results showed that there were no cases of acute exacerbation of ILD or death within 90 days following ablation. The most frequent adverse events were pneumothorax requiring a chest tube, hemoptysis, pneumothorax not requiring a chest tube, and bronchopleural fistula. 74% of the adverse events were classified as grade I or II. The median hospital length of stay was 1 day.<br /><br />The study concludes that thermal ablation, including cryoablation, radiofrequency ablation, and microwave ablation, is a promising treatment option for patients with ILD and early-stage NSCLC. This minimally invasive procedure showed no cases of acute exacerbation of ILD or death within 90 days following treatment. The findings suggest that thermal ablation could be an effective and safe treatment for this patient population.
