This study compared the use of non-contrast chest CT with contrast chest CT for surveillance in patients who had undergone curative resection for lung cancer. Current guidelines recommend different intervals and types of CT scans for surveillance, but there is limited clinical evidence supporting these recommendations. The objective of this study was to compare the outcomes of patients who underwent non-contrast CT versus contrast CT after 2 years from curative resections.<br /><br />The study included patients who had undergone curative lung cancer surgery between January 2011 and September 2017. Patients with recurrence or follow-up loss within 24 months, multiple lung cancer operations, absence of CT follow-up during the study period, and patients with chronic kidney disease or contrast allergy were excluded. The final analysis included 1995 patients, with 620 in the non-contrast CT group and 1375 in the contrast CT group.<br /><br />The results showed that there was no significant difference in overall survival between the non-contrast CT group and the contrast CT group. This finding was consistent among patients with stage I lung cancer and those with stage II/III lung cancer. The study suggests that non-contrast CT can be a feasible option for surveillance in patients who did not experience recurrence within the first 2 years after surgery.<br /><br />The study's limitations include its retrospective nature and potential selection bias. Additionally, the study did not assess the diagnostic accuracy of non-contrast CT compared to contrast CT for detecting recurrent disease. Further research is needed to validate these findings and determine the optimal surveillance strategy for patients who have undergone curative resection for lung cancer.
