EP07.01. Long Term Outcomes beyond Five Years after Pulmonary Resection for Non Small Cell Lung Cancer - PDF(Slides)

A study conducted by Tetsuya Mizuno and colleagues aimed to identify candidates for long-term surveillance after non-small cell lung cancer (NSCLC) resection. The study included 978 NSCLC patients who underwent pulmonary resection between 2002 and 2015 and survived without recurrence for five years. The researchers analyzed various variables including age, sex, smoking history, preoperative serum CEA levels, surgical procedure, histology, pN status, presence of adjuvant therapy, development of recurrence, and development of metachronous malignancies.<br /><br />The study found that late recurrence beyond five years was observed in 1.8-11% of patients with resected NSCLC. The overall survival rate beyond five years post-surgery was 89.1% and the disease-free survival rate was 94.4%. Factors such as pN status, preoperative serum CEA levels, and histology were significantly associated with recurrence-free survival beyond five years post-surgery.<br /><br />In terms of metachronous malignancies, the study found that the incidence of late metachronous malignancies beyond five years post-surgery was 16.3%. The most common primary organs affected were the lung, stomach, colorectum, and urinary tract.<br /><br />Based on these findings, the researchers concluded that long-term surveillance for recurrence beyond five years may have limited usefulness, especially in patients with pN0 adenocarcinoma and/or non-adenocarcinoma. The study highlights the importance of considering individual patient characteristics when determining the need for long-term surveillance after NSCLC resection. Further research is needed to better understand the long-term outcomes and factors affecting them in NSCLC patients.
