Pleomorphic carcinoma in the lung is a rare type of cancer that makes up 0.3% of invasive non-small lung cancers. It is usually found in the right lung's upper lobe and tends to be peripheral with invasion of the chest wall. The prognosis for pleomorphic carcinoma is worse than other non-small cell lung cancers, with metastases occurring relatively quickly after surgery. Preoperative diagnosis is not always accurate, and the definitive diagnosis is usually determined from the resected specimen.<br /><br />A retrospective analysis was conducted on 10 patients who underwent lobectomy or a more extensive resection for pleomorphic carcinoma between September 2012 and February 2023. The study looked at their clinical and pathological characteristics to determine the optimal postoperative surveillance and treatment strategy.<br /><br />The results showed that the median age of the patients was 76, with seven males and three females. The median tumor size was 39.5 mm on preoperative computed tomography (CT) and 48.5 mm in the resected specimen. There were cases where lymph node metastases were found, and in some cases, the cancer progressed from N1 to N2. Recurrence occurred within six months after surgery in some patients, highlighting the need for more frequent surveillance.<br /><br />Currently, there are no specific guidelines for treating pleomorphic carcinoma in the lung, so the approach is based on guidelines for non-small cell lung cancer. However, the study suggests that an individualized approach may be more beneficial, including evaluating mutation status and PD-L1 expression in the resected specimen and shorter follow-up intervals for imaging to detect recurrence and start treatment promptly.<br /><br />The study emphasizes the aggressive nature of pleomorphic carcinoma and the need for distinct guidelines for this type of cancer. The researchers also anticipate the results of ongoing clinical studies, such as the CHANCE trial, to provide further insights into the management of pleomorphic carcinoma in the lung.
