EP07.02. Hypofractionated Radiation Therapy vs Stereotactic Body Radiation Therapy for Ultra-Central Lung Tumors - PDF(Abstract)

This study compared two radiation therapy techniques, hypofractionated (HF) radiation therapy and stereotactic body radiation therapy (SBRT), for the treatment of ultra-central lung tumors (UCLT). The researchers conducted a retrospective study on patients who underwent radiation therapy for UCLT between 2008 and 2018. The study included 142 patients, with 65 receiving HF and 77 receiving SBRT. <br /><br />The results showed that there were no significant differences in overall survival (OS) between the two groups. However, progression-free survival (PFS) favored the SBRT group. The HF group had higher rates of grade 1-2 pneumonitis and esophagitis compared to the SBRT group. There was also one grade 3 toxicity event and one grade 5 event in the HF group, but no grade 3 events attributable to tumor or treatment in the SBRT group. <br /><br />The study concluded that both HF and SBRT are safe and viable treatments for UCLT. However, PFS appeared to be better for the SBRT group. The researchers noted that baseline differences and selection bias in the retrospective study limit clear conclusions. Patients with larger tumors and closer proximity to central anatomy seemed to fare well with HF. The observed grade 3 events may also be a result of the selection bias leading to HF treatment. The study suggests that additional data are needed to better define the optimal dose selection for UCLT.
