EP07.02. The Role of Online MR-guided Multi-Fraction Stereotactic Ablative Radiotherapy in Lung Tumors - PDF(Slides)

This study aimed to evaluate the potential benefits, changes in pulmonary function, and clinical outcomes of online adaptive MR-guided multi-fraction stereotactic ablative radiotherapy (SAbR) in the treatment of lung tumors. The study included 45 patients with 59 lung tumors who underwent online adaptive MR-guided SAbR. The primary endpoints were local control (LC) and overall survival (OS).<br /><br />The results showed that the PTV coverage by the prescribed dose increased in 94.6% of all fractions, with a median increase in PTV V100% of 5.6%. PTV D95% and D98% increased in 93% and 95% of all fractions, respectively. The PTV D95% (BED10) increased by a mean of 9.6 Gy. At a median follow-up of 21.4 months, the 1- and 2-year OS rates were 85.6% and 67.1%, and the 1- and 2-year LC rates were 94.8% and 91.1%.<br /><br />The study found that online adaptive MR-guided SAbR is an effective and safe treatment option for lung tumors. It achieved excellent local control rates and significantly improved dose coverage of target volumes. There was no significant decline in pulmonary function parameters. The most common grade 2 treatment-related toxicities were radiation pneumonitis and dyspnea.<br /><br />In conclusion, this study demonstrated that online adaptive MR-guided multi-fraction SAbR is a beneficial treatment option for lung tumors, providing excellent local control rates and improved dose coverage. It is a safe treatment that does not result in significant decline in pulmonary function.
