EP07.03. Negative Impacts of Mediastinal Lymph Node Dissection Forstage I Ground Glass Opacity Pulmonary Lesions - PDF(Abstract)

This study aimed to evaluate the clinical impacts of mediastinal lymph node dissection (MLND) among patients with ground glass opacity (GGO) lesions in stage I non-small cell lung cancer. MLND is commonly performed in lung cancer surgery to remove lymph nodes in the mediastinum. However, its necessity for GGO lesions with certain characteristics has been questioned. The study included 138 patients with GGO lesions and a consolidation tumor ratio (CTR) between 0.3 and 0.7. They were divided into two groups: limited MLND (3 N2 stations) and extended MLND (more than 3 N2 stations). The researchers compared the recurrence-free survival (RFS) and overall survival (OS) between the two groups using Kaplan-Meier curves. They also analyzed the risk factors for RFS, OS, and postoperative complications (PoC) using statistical analyses. <br /><br />The results showed that 27.5% of patients underwent extended MLND. There were no significant differences between the two MLND groups in terms of demographic characteristics, metabolic activity, CTR, and pathologic diagnosis. However, the extended MLND group had a larger solid diameter and more patients undergoing lobectomy. The occurrence of postoperative complications was higher in the extended MLND group. The Kaplan-Meier survival curves did not show a significant difference in clinical outcomes between the two groups. However, extended MLND was identified as a significant risk factor for the development of postoperative complications. <br /><br />Based on these findings, the authors suggest that the current MLND strategy for lung cancer may not be appropriate for GGO lesions with a CTR between 0.3 and 0.7 in terms of postoperative complications and long-term outcomes. They emphasize the need for further discussion and research on MLND for these part-solid lesions.
