A study conducted by Wongi Woo and colleagues from Gangnam Severance Hospital in Seoul, Republic of Korea, aimed to evaluate the impact of mediastinal lymph node dissection (MLND) for ground glass opacity (GGO) lesions in patients with stage I pulmonary adenocarcinoma. The study, which involved a retrospective review of 476 patients from 2013 to 2019, analyzed 138 patients with consolidation tumor ratio (CTR) between 0.3 and 0.7.<br /><br />The results of the study indicated that extended MLND for GGO lesions with CTR between 0.3 and 0.7 may have negative effects on early postoperative outcomes without providing significant long-term oncologic benefits. The study suggests that the general MLND strategy should be reconsidered for patients with GGO-containing lesions.<br /><br />The study also assessed the risk factors for postoperative complications and found that male gender and extended N2 MLND were associated with a higher risk. However, cardiovascular disease history, smoking history, and sublobar resection did not show a significant association with postoperative complications.<br /><br />The authors emphasize the need for further studies and discussions among surgical societies to develop appropriate MLND approaches based on tumor characteristics. They suggest that a solid background is necessary to guide the application of MLND strategies in patients with GGO lesions.<br /><br />In conclusion, the study highlights the potential negative effects of extended MLND for GGO lesions with CTR between 0.3 and 0.7 in terms of early postoperative outcomes. However, further research is needed to determine the optimal MLND approaches for patients with GGO-containing lesions based on tumor characteristics.
