EP07.04. Clinicopathological Features and Postoperative Prognosis of Bubble-like Appearance Type Lung Adenocarcinoma - PDF(Abstract)

This study aimed to evaluate the clinicopathological features and postoperative prognosis of patients with bubble-like appearance (BLA) type lung adenocarcinoma. BLA-type lung adenocarcinomas are sometimes diagnosed as old inflammatory lesions and may be overlooked. The researchers retrospectively reviewed the data of 130 patients with BLA-type lung adenocarcinoma who underwent complete surgical resection between January 2010 and December 2019. <br /><br />The median tumor size on radiological examination was 3.5 cm, and most patients had stage I disease. The median observation period was 61.9 months. The 5-year recurrence-free survival (RFS) rate was 78.5%, and the 5-year overall survival (OS) rate was 92.5%. However, the rates differed based on the pathological stage, with stage I having a higher RFS and OS compared to stage II-III. <br /><br />EGFR mutations were found in 63.8% of the cases. Recurrence was observed in 22 cases, with the majority occurring in patients with lymph node involvement (pN1-2). Multivariable analysis revealed that smoking history, pN factor, and vascular invasion were significant factors influencing RFS. Age and pN factor were significant factors affecting OS. <br /><br />The study concluded that although many BLA-type lung adenocarcinomas were diagnosed at an early stage, careful attention is needed when lymph node metastasis occurs as it is associated with a high recurrence rate. Additionally, patients with these types of lesions had a high rate of EGFR mutation. This information can help guide clinical decision-making and treatment planning for patients with BLA-type lung adenocarcinoma.
