Pdf Summary
This study analyzed the factors associated with postoperative recurrence in early non-small cell lung cancer (NSCLC) patients with EGFR mutations using Korean national population data. The researchers obtained data from the Korean Association for Lung Cancer Registry and evaluated 3,176 patients who underwent curative resection. The patients were staged according to the TNM classification, and those who experienced postoperative relapse within one year of resection were classified as early recurrence patients.

Among the patients with early NSCLC and EGFR mutations, 44 (5.1%) showed early recurrence, and 121 (14.0%) showed late recurrence. Multivariate analysis revealed that N stage, pleural invasion status, and operation type were independent significant factors associated with postoperative relapse in the EGFR positive subgroup.

The study suggests that patients with a large tumor burden or at risk of incomplete resection may exhibit early postoperative relapse and could benefit from postoperative adjuvant EGFR tyrosine kinase inhibitors (TKI). The findings provide real-life data on the subgroup of patients at high risk of relapse and in need of adjuvant EGFR TKI treatment. These insights can contribute to the management and treatment of early NSCLC with EGFR mutations.
