This session at the WCLC 2023 conference focused on the progress in understanding the pathology of early-stage non-small cell lung cancer (NSCLC) with a specific emphasis on pure ground-glass nodules (GGN). The detection of GGN has increased with the development of lung cancer screening programs, but the pathologic characteristics of these nodules have not been well-defined. <br /><br />The authors conducted a systematic review and meta-analysis of studies published until June 2022 that included pathological reports of pure GGN. They included a total of 3,845 cases of pure GGN that underwent surgical resection from 24 studies. The majority of patients (60%) underwent sublobar resection. <br /><br />The results showed that the proportion of invasive adenocarcinoma (IA) in resected pure GGN cases was 27%. Furthermore, 50% of the cases had non-lepidic predominant patterns. The proportions of atypical adenomatous hyperplasia (AAH), adenocarcinoma in situ (AIS), and minimally invasive adenocarcinoma (MIA) were 11%, 36%, and 24%, respectively. <br /><br />Among the studies with available clinical outcome data, there were no recurrences or metastases following surgery, except in one study. This suggests that surgical resection is an effective treatment approach for pure GGN, even in cases with radiologically favorable appearances. <br /><br />In conclusion, the authors found that the diagnosis of invasive adenocarcinoma is relatively common in cases of pure GGN, with more than half of them having an invasiveness component. Therefore, surgical resection should be considered for all patients with pure GGN, regardless of radiological appearance. This study contributes to the understanding of the pathology of early-stage NSCLC and provides important insights for clinical decision making.
