Pdf Summary
This study focuses on lymphatic vascular invasion (LVI) in patients undergoing surgical resection for non-small cell lung cancer (NSCLC) and its impact on overall survival. The researchers used data from the National Cancer Database (NCDB) to identify patients who underwent lobectomy or sub-lobar resection and had lymph nodes examined. Propensity score matching (PSM) was used to analyze the impact of LVI on overall survival, adjusting for other factors.

The study found that LVI status was reported in 91.5% of eligible patients, with 21.3% of them testing positive for LVI. LVI was independently associated with a significant decrease in overall survival. Academic medical centers, institutions with larger populations, and the mid-Atlantic region had higher rates of LVI tumors.

The impact of LVI on overall survival was consistent across different node stages and tumor grades. Patients undergoing lobectomy with adequate lymph node dissection had a decreased overall survival if they had LVI-positive tumors. Similarly, for patients undergoing sub-lobar resection, LVI had an impact on overall survival when a sufficient number of lymph nodes were examined.

The study highlights the variability in LVI rates based on hospital location/type and population. It suggests that LVI should be standardized and considered as a staging variable, and that it may be a determining factor for systemic therapy in certain patient groups.

In conclusion, LVI in NSCLC patients undergoing surgical resection is associated with a decrease in overall survival. The study emphasizes the importance of considering LVI as a staging variable and incorporating it into treatment decisions for specific patient populations.
