A study conducted at Asan Medical Center in Seoul, Korea, evaluated the outcomes of stereotactic body radiotherapy (SBRT) in patients with early-stage non-small cell lung cancer (NSCLC) and interstitial lung disease (ILD). ILD is a group of diseases characterized by inflammatory or fibrotic pathologies that increase the risk of developing lung cancer. SBRT is commonly used to treat early-stage NSCLC in patients who cannot undergo surgery, but it carries a higher risk of lung toxicity in ILD patients. <br /><br />The study analyzed the medical records of 57 patients with ES-NSCLC and ILD who underwent SBRT between 2009 and 2019. The severity of ILD was evaluated using the ILD-GAP model combined with CT features. The results showed that patients with ES-NSCLC and ILD had a poor prognosis and experienced a high incidence of treatment-related lung toxicity. <br /><br />When analyzing patterns of recurrence, it was found that the most common site of recurrence was the lung, and the 2-year freedom from local progression rate was 70.3%. The overall survival rate at 2 years was 50.9% and the progression-free survival rate was 32.5%. <br /><br />Eleven patients experienced grade 3 pneumonitis or ILD aggravation, while four patients had grade 5 aggravation. However, no clinical factors were significantly associated with the occurrence of pneumonitis. <br /><br />The study concludes that although patients with ES-NSCLC and ILD have a poor prognosis and high risk of lung toxicity following SBRT, options for cancer-directed local treatments are limited. Therefore, despite the associated risks, it may be necessary to consider SBRT as a treatment option for these patients due to the limited alternatives available for disease progression.
