EP07.05. Postoperative Recurrence Pattern and the Role of Adjuvant Therapy in Radiological Solid-Predominant Part-Solid Lung Adenocarcinoma - PDF(Slides)

A study was conducted to investigate the recurrence pattern and the role of adjuvant therapy in solid-predominant lung adenocarcinoma. Radiological part-solid nodules (PSNs) containing both solid and ground-glass opacity (GGO) components were found to have a greater risk of recurrence and death compared to GGO-predominant nodules. The study included patients with clinical N0 solid-predominant PSNs who underwent curative surgery between 2008 and 2015. Out of 462 eligible patients, 88 (19.0%) experienced postoperative recurrence, with 26 (5.6%) having locoregional-only recurrence, 38 (8.2%) having distant-only recurrence, and 24 (5.2%) having combined recurrences. The 5-year overall survival was significantly lower in patients with recurrence compared to those without recurrence. Lymph node involvement, lymphovascular invasion, and larger solid component diameter were identified as independent risk factors for postoperative recurrence. The use of tyrosine kinase inhibitors and late recurrence were predictors of favorable outcomes after recurrence. The study concluded that postoperative recurrence is not uncommon in solid-predominant PSNs and has a significant impact on long-term survival. The recurrence pattern and adjuvant treatment strategy were found to predict survival after recurrence, highlighting the importance of ongoing monitoring and targeted therapy for patients with recurrent disease.
