EP07.05. Spontaneous Ventilation Video-assisted Thoracoscopic Surgery for Super-elderly NSCLC Patients: a Noninferiority Study. - PDF(Slides)

This study aims to assess the feasibility and safety of spontaneous ventilation (SV) video-assisted thoracoscopic surgery (VATS) in super-elderly patients with non-small cell lung cancer (NSCLC). A total of 324 NSCLC patients aged over 80, who underwent either SV-VATS or mechanical ventilation (MV)-VATS, were included in the study. Propensity score matching was performed to balance baseline characteristics between the two groups. <br /><br />The study compared various intraoperative and postoperative outcomes, including surgery time, anesthesia time, post-anesthesia care unit (PACU) time, chest tube duration, the total volume of chest drainage, bleeding, urinary and liquid infusion volume, ventilation time, resuscitation time, postoperative length of stay, neutrophil-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-in?ammation index (SII), age-adjusted Charlson comorbidity index (ACCI), and complications in PACU. No statistically significant differences were observed between the SV-VATS and MV-VATS groups in most of these outcomes. However, the SV-VATS group had shorter PACU time and resuscitation time compared to the MV-VATS group.<br /><br />Long-term survival outcomes were also assessed, and the median follow-up time was 23.8 months. The study found that patients who underwent SV-VATS had comparable overall survival (OS) and disease-free survival (DFS) compared to those who underwent MV-VATS.<br /><br />In conclusion, this study suggests that SV-VATS is feasible and safe in super-elderly NSCLC patients. The SV-VATS group had shorter PACU and resuscitation times without compromising other outcomes or long-term survival. These findings contribute to the growing evidence supporting the use of SV-VATS in this patient population.
