EP08.02. Clinicopathological Analyses for Predicting Survival After Salvage Surgery for Initially Unresectable Lung Cancer - PDF(Slides)

This study aimed to evaluate the prognosis and identify clinicopathological predictors of postoperative survival in patients who underwent salvage surgery for initially unresectable lung cancer. The study included 25 patients who underwent salvage surgery out of a total of 2096 patients who underwent surgical resection for primary lung cancer from 2010 to 2022. The patients had locoregionally residual or relapsed lesions after definitive radiotherapy and/or systemic therapy.<br /><br />The study evaluated the pathological changes in response to preoperative therapy and analyzed postoperative survival according to clinicopathological features. The median age of the patients was 61 years, and the majority were male with a smoking history. The extent of resection varied, with lobectomy being the most common. The perioperative outcomes were favorable, with no postoperative mortality and low complication rates.<br /><br />The study found that a high proportion of stroma in the resected specimen was associated with favorable overall survival. In contrast, the presence of a major pathological response (MPR) did not significantly impact survival. This suggests that the proportion of stroma may be a useful prognostic indicator after salvage surgery.<br /><br />The authors conclude that salvage surgery for initially unresectable lung cancer can be safely performed and has relatively favorable outcomes. They suggest that the proportion of stroma in the resected specimen may serve as a useful prognostic indicator and indicate the need for proactive postoperative therapies. This study provides valuable insights into the clinicopathological factors that may predict survival after salvage surgery for unresectable lung cancer.
