Pdf Summary
This study compared the long-term survival outcomes of patients with locally advanced lung cancer who underwent video-assisted thoracoscopic surgery (VATS) versus thoracotomy. The study included 512 lung cancer operations, with 117 for stage II/III lung cancer. Of these, 62 patients underwent VATS and 55 underwent thoracotomy. The primary outcome measure was overall survival (OS), and the secondary outcome measure was disease-free survival (DFS).

The results showed that the mean OS time of the VATS group was 150.8 months, compared to 115.1 months in the thoracotomy group. The OS rates at one, three, and five years after surgery were higher in the VATS group compared to the thoracotomy group. The median DFS time of the VATS group was also longer than the thoracotomy group. However, there was no significant difference in 5-year OS or DFS between the two groups.

These findings suggest that VATS can achieve similar prognosis as thoracotomy for locally advanced lung cancer. The study highlights the importance of considering patient-reported outcomes that reflect the postoperative recovery process when selecting the surgical approach. This research contributes to the understanding of the impact of VATS and thoracotomy on the long-term survival of patients with locally advanced lung cancer.
