EP08.02. Management of Inoperable Locally Advanced Stage III ALK fusion Positive NSCLC: A Retrospective Multi-institutional Analysis - PDF(Slides)

This retrospective multi-institutional analysis aimed to determine the optimal management for inoperable locally advanced ALK fusion positive stage III non-small cell lung cancer (NSCLC). The study analyzed data from a prospectively maintained database at the Shanghai Chest Hospital between 2017 and 2022.<br /><br />A total of 169 patients were eligible for analysis out of 58,202 patients treated during the study period. The patients were stratified into three groups based on the therapy they received: chemotherapy alone, chemoradiation (CRT), and ALK-TKI (with or without chemotherapy). The baseline characteristics of the patients were generally balanced between the three groups, except for more smokers in the chemotherapy arm.<br /><br />The analysis showed that the median time to disease progression was 17.8 months for patients who received ALK-TKI, 14.6 months for those who underwent CRT, and 8.6 months for those who received chemotherapy alone. The 1-year progression rate was 68.8% for ALK-TKI, 60.8% for CRT, and 31.4% for chemotherapy alone.<br /><br />The study concluded that the prognosis of patients who received ALK-TKI as a first-line therapy was superior to those receiving chemotherapy or chemoradiation. However, further studies are needed to understand the efficacy of ALK-TKI as a first-line or maintenance therapy for these patients.<br /><br />The authors acknowledged the contributions of individuals from several organizations for providing medical insights and publication suggestions.
