Video-assisted thoracoscopy (VATS) is a preferred surgical technique for early-stage non-small cell lung cancer (NSCLC) due to its benefits such as faster recovery and lower pain. However, neoadjuvant therapy (NT) has been considered a relative contraindication to VATS in the past. This study aimed to evaluate the feasibility and safety of VATS in NSCLC patients who received NT.<br /><br />The study included 306 patients who underwent VATS lung resection between January 2011 and December 2021. The patients were divided into two groups: Group 1 (patients who did not receive NT) and Group 2 (patients who received NT). The surgical outcomes and complications were compared between the groups.<br /><br />In Group 2, NT consisted of chemotherapy, chemoradiotherapy, and immunotherapy. The conversion to thoracotomy was similar in both groups. Complications were detected in 33% of patients after surgery, with the most common being prolonged air leak. Mortality occurred in 1.6% of patients. There was no significant difference in recurrence or metastasis between the groups.<br /><br />The study showed that VATS can be safely performed in NSCLC patients who have received NT, with similar oncological outcomes. The results suggest that NT can reduce tumor size and increase the frequency of VATS, indicating a new benefit of neoadjuvant therapy.<br /><br />In a case example, a 73-year-old male with NSCLC received neoadjuvant chemotherapy before VATS left upper lobectomy. The tumor size decreased, and the patient had a successful surgery without complications.<br /><br />In summary, this study suggests that VATS can be a safe option for NSCLC patients who have received neoadjuvant therapy. Neoadjuvant therapy can reduce tumor size and increase the feasibility of VATS.
