EP08.02. Robot-Assisted Thoracic Surgery for Locally Advanced Non-small Cell Lung Cancer in a Japanese Single Institution - PDF(Abstract)

This study examined the feasibility and safety of robot-assisted thoracic surgery (RATS) for locally advanced non-small cell lung cancer (NSCLC) in a Japanese single institution. A total of 167 cases of RATS for primary lung cancer were performed between April 2018 and December 2022, and 25 cases of locally advanced NSCLC were resected using RATS since July 2021. The surgical outcomes of these 25 patients were compared with the outcomes of 142 patients in a control group. <br /><br />All 25 cases of RATS for locally advanced NSCLC were completed successfully without the need for conversion to open thoracotomy or conventional video-assisted surgery. The surgical procedures included single lobectomy in 24 patients and lobectomy with adjacent segmental resection in one patient. Extended procedures such as chest wall resections, major vessel resections and reconstructions, mediastinal tissue resection, angioplasty of pulmonary artery, and bronchoplasty were also performed in some patients. The mean operation time for the advance group was 289 minutes, and the intraoperative blood loss was 164 grams. There were no mortalities in either the advance or control groups. <br /><br />Postoperative complications included prolonged air leakage in three cases, empyema in one case, and bronchopleural fistula in one case. There was no significant difference in the occurrence of postoperative major complications between the advance and control groups. <br /><br />In conclusion, RATS for locally advanced NSCLC is a feasible and safe surgical approach. Robotic technology may aid in performing more complicated surgical procedures needed in the multimodality treatment strategy for locally advanced NSCLC.
