EP08.02. Robot-Assisted Thoracic Surgery for Locally Advanced Non-small Cell Lung Cancer in a Japanese Single Institution - PDF(Slides)

Robot-assisted thoracic surgery (RATS) has become a viable option for the treatment of locally advanced non-small cell lung cancer (NSCLC). A study conducted at Fukuoka University in Japan evaluated the feasibility and safety of RATS for these advanced tumors and compared the surgical outcomes with a control group of patients.<br /><br />The study included a total of 167 cases of RATS anatomic resection for NSCLC, with 25 cases classified as locally advanced (stage IIB or higher). The surgical procedures included lobectomy and segmentectomy, with additional extended procedures in some cases. The standard robot approach involved utilizing the Da Vinci Xi system, with specific variations depending on the side and the surgical procedure.<br /><br />The results showed that RATS for locally advanced NSCLC was associated with favorable surgical outcomes. The operative time was comparable between the advanced group and the control group, while blood loss was significantly higher in the advanced group. There were no conversions to open or VATS surgery, and no mortality was reported in either group. However, the advanced group had a higher rate of morbidity, with complications such as prolonged air leakage, empyema, and bronchopleural fistula.<br /><br />Pathological findings after surgery revealed a range of tumor classifications, with some cases showing no residual tumor cells. Adjuvant chemotherapy was administered in 12 cases.<br /><br />The study concluded that robotic technology can facilitate complex surgical procedures required for locally advanced NSCLC while maintaining minimal invasiveness. RATS for these advanced tumors was found to be feasible and safe, with acceptable surgical outcomes. These findings contribute to the growing body of evidence supporting the use of RATS in the treatment of locally advanced NSCLC.
