EP08.02. Trimodality Treatment of Superior Sulcus non-Small Cell Lung Cancer. An Institutional Series of 47 Consecutive Patients. - PDF(Abstract)

This study focused on the treatment of superior sulcus tumors (SST) in non-small cell lung cancer patients. The standard treatment for SST is concurrent chemoradiation followed by surgery, but there is limited clinical experience in this area. The study aimed to present the results of a large series of patients treated with this approach.<br /><br />The study included 48 patients with confirmed SST, who received preoperative radiotherapy and two cycles of platinum-based chemotherapy. After completing chemoradiation, the patients underwent pulmonary resection. One patient did not undergo surgery due to brain metastases.<br /><br />The treatment was well tolerated, with no toxicity-related deaths. Some patients experienced grade 3-4 side effects, most commonly neutropenia. Postoperative complications occurred in 36.2% of patients, and the 90-day mortality rate was 2.1%.<br /><br />The median follow-up was 64.7 months. The three- and five-year overall survival rates were 43.6% and 33.5%, respectively. The three- and five-year recurrence-free survival rates were 42.1% and 32.4%, respectively.<br /><br />Thirteen patients had a complete pathological response, while 22 had a major pathological response. Patients with complete tumor regression had a five-year overall survival of 52.7%. Age below 70 years, complete resection, pathological stage, and response to induction treatment were predictive factors of long-term survival.<br /><br />In conclusion, the study found that concurrent chemoradiation followed by surgery is a safe method with satisfactory outcomes in the treatment of SST.
