EP09.01. Is Palliative Thoracic Radiation Beneficial in Stage IV Non-small Cell Lung Cancer (NSCLC) In the Era of Modern Systemic Therapy? - PDF(Slides)

This study examines the outcome of stage IV Non-small cell lung cancer (NSCLC) and the impact of palliative thoracic radiation therapy in patients with this disease. The survival rates for stage IV NSCLC have historically been poor, with only 5% of patients surviving at 5 years. However, recent advances in systemic therapy have led to an increase in overall survival rates of up to 20%. As systemic therapy improves, the role of palliative therapy, such as thoracic radiation, also evolves.<br /><br />The study analyzed data from 1,107 patients diagnosed with stage IV NSCLC between 2002 and 2018. The median age of the patients was 65 years. The analysis showed that older age, poorly differentiated tumor, and NSCLC histology were adverse factors for survival. On the other hand, the use of chemotherapy and palliative thoracic radiation were beneficial for survival.<br /><br />The study also looked at 3- and 5-year conditional survival rates for patients who were alive at 1 year and in the lowest risk category. The 3-year conditional survival rate was 71%, and the 5-year conditional survival rate was 61%.<br /><br />Overall, the study found measurable progress in overall survival for stage IV NSCLC over the years. Palliative thoracic radiation therapy showed a statistically significant survival benefit, especially when combined with improved systemic therapy. As systemic therapy continues to improve, further research is needed to define the role of thoracic radiation in improving survival outcomes for stage IV NSCLC patients. Additionally, the study suggests that modeling based on clinical factors and treatment received can help identify patients who will benefit the most from therapy.<br /><br />In conclusion, this study highlights the impact of palliative thoracic radiation therapy for stage IV NSCLC patients in the context of improved systemic therapy. Future research should focus on prospective studies to further define the role of thoracic radiation and improve survival outcomes for this patient population.
