Pdf Summary
This research presented at the WCLC 2023 conference aimed to evaluate the therapeutic effects of combining epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) with radiotherapy on the prognosis of patients with stage IV lung adenocarcinomas (LUAD) that have specific genetic mutations. The study analyzed clinical data from 177 stage IV LUAD patients who received the combination therapy at Shandong Cancer Hospital.

The results revealed that patients with exon 19 deletion (19-Del) and exon 21 L858R mutation (21-L858R), both exhibited similar clinical characteristics. The median overall survival (mOS) for all patients was 32.0 months. Multivariate analysis identified EGFR mutations and thoracic radiotherapy as independent factors predicting overall survival.

Subgroup analysis showed that patients with 19-Del who received EGFR-TKIs plus bone metastasis radiotherapy or thoracic radiotherapy had a longer overall survival compared to those with 21-L858R (mOS: 34.7 vs. 25.1 months; 51.0 vs. 29.6 months, respectively). However, this difference was not observed in patients with brain metastasis radiotherapy.

In conclusion, although there were no significant differences in clinical characteristics between patients with 19-Del and 21-L858R, those with 19-Del had a longer overall survival after receiving EGFR-TKIs in combination with radiotherapy. This research suggests that combining EGFR-TKIs with radiotherapy may have a positive impact on the prognosis of stage IV LUAD patients with specific genetic mutations.
