EP09.02. Long-term Efficacy of Tumor Treating Fields (TTFields 150 kHz) in metastatic NSCLC: A Case Report - PDF(Slides)

This case report presents the long-term efficacy of Tumor Treating Fields (TTFields 150 kHz) therapy in a 58-year-old female with metastatic non-small cell lung cancer (NSCLC). TTFields are electric fields that disrupt cancer cell division, and they are delivered to the cancer region using a lightweight, battery-operated device. The patient underwent TTFields therapy together with atezolizumab as a second-line treatment. <br /><br />The results of the LUNAR study, a phase 3 clinical trial, showed a significant extension in overall survival (OS) with TTFields compared to standard of care (SOC). After a minimum follow-up of 12 months, the median OS was 13.2 months with TTFields plus SOC compared to 10.0 months with SOC alone. TTFields therapy also improved OS compared to immunotherapy alone, with a median OS of 18.5 months versus 10.6 months. The majority of TTFields-related adverse events (AEs) were grade 1 and 2 local skin irritation, with only 6% of patients reporting a grade 3 AE. There were no grade 4 toxicities or deaths attributed to TTFields therapy.<br /><br />During the treatment, the patient experienced grade 2 pruritus beneath the TTFields arrays, which was managed with skin barrier cream and topical glucocorticoids. The patient continued TTFields therapy for 38 months with a partial response of the disease, as confirmed by CT scans.<br /><br />This case report highlights the breakthrough in the treatment of metastatic NSCLC patients who have not responded to previous therapies. TTFields therapy with immunotherapy shows potential as a viable long-term treatment strategy. The proper management of skin adverse events is crucial to maintaining treatment efficacy and patient well-being. Overall, TTFields therapy offers renewed hope to patients facing metastatic NSCLC following unsuccessful platinum-based therapies.
