Pdf Summary
A study presented at WCLC 2023 explored the efficacy of combining local ablative therapy (LAT) with immune checkpoint inhibitors (ICIs) in advanced non-small cell lung cancer (NSCLC) patients who experienced oligo-progression after ICIs treatment. Oligo-progression refers to the progression of a limited number of tumor lesions while the majority remain stable. The researchers retrospectively analyzed data from 94 patients who received LAT plus ICIs and 91 patients who received ICIs alone.

The results showed that patients in the LAT group had a significantly longer time to next treatment (TTNT) and overall survival (OS) compared to the non-LAT group. LAT also induced abscopal effects, which are systemic responses seen outside of the treated area. In the LAT group, 9.7% of patients experienced abscopal effects, while there were no abscopal effects in the non-LAT group. Additionally, patients with abscopal responses or stable disease had significantly improved TTNT and OS compared to those with progression in distant lesions.

Further analysis revealed that patients with abscopal effects or more than a 10% reduction in distant lesions benefited the most from the combined treatment. These patients had longer TTNT and OS compared to those without these responses. It was also observed that patients with abscopal responses had milder decreases in lymphocytes compared to those with progression in distant lesions.

The study concluded that adding LAT to ICIs significantly improved abscopal responses and outcomes in oligo-progressive NSCLC patients, with manageable toxicities. By overcoming resistance to ICIs, the combination therapy of LAT and ICIs could be a valuable treatment option for advanced NSCLC patients. However, some important details of the study were missing or not specified in the summary.
