EP09.05. Changes in Pulmonary Function Following Single and Multi-Fraction Stereotactic Body Radiotherapy for Pulmonary Oligometastatic Disease - PDF(Slides)

The SAFRON II trial compared single fraction (SF) stereotactic body radiotherapy (SBRT) and multi-fraction (MF) SBRT in patients with pulmonary oligometastases. The trial found no differences in efficacy or toxicity between the two treatment arms. However, this analysis aimed to assess the differences in pulmonary function tests (PFTs) between the arms and evaluate the relationship between changes in PFTs and the number and volume of targets treated.<br /><br />The study included 90 patients with 133 pulmonary oligometastases. 44 patients were randomized to the SF arm and 43 to the MF arm. There were no significant differences in absolute or relative PFT measures of forced expiratory volume in 1 second (FEV1) or diffusing capacity of the lung for carbon monoxide (DLCO) between the two arms.<br /><br />The analysis also revealed that as the number of targets increased, FEV1 at 12 months post-treatment decreased by approximately 100ml per lesion. Similarly, as the planning target volume (PTV) size increased, FEV1 at 12 months decreased by approximately 3ml per 10ml of PTV treated. There were no significant changes in DLCO associated with the number of targets or PTV volume.<br /><br />Overall, this analysis suggests that there were no differences in PFTs between SF and MF SBRT in the treatment of pulmonary oligometastases. However, it highlights that as the number of targets and PTV size increased, there was a reduction in FEV1 at 12 months post-treatment. These findings provide important insights into the potential effects on pulmonary function following SBRT for pulmonary oligometastatic disease.
