Pdf Summary
Elderly patients with advanced lung cancer were found to be less likely to choose a chemotherapy regimen that includes cytotoxic anticancer agents compared to non-elderly patients. Specifically, regimens containing pemetrexed (PEM) were used less frequently in elderly patients. However, ICI monotherapy was used more frequently in the elderly. The study also found that there were no significant differences in Progression-Free Survival (PFS) or Overall Survival (OS) based on renal function.

The researchers conducted a retrospective study on patients diagnosed and initially treated for non-squamous non-small cell lung cancer (NSCLC) between January 2017 and July 2022. They aimed to examine the difference in regimen selection and its impact on survival based on age and renal function.

Although elderly patients had worse renal function and there were differences in regimen selection, there were no differences in PFS or OS. The use of cytotoxic anticancer agents with immune checkpoint inhibitors (ICI) as first-line chemotherapy was significantly lower in elderly patients compared to non-elderly patients.

The study also found that the use of pemetrexed (PEM) was less frequent in elderly patients. However, ICI monotherapy was more common in the elderly. Elderly patients did not show differences in PFS and OS based on renal function. This could be attributed to the less frequent use of PEM, the trend towards more frequent ICI use, and the relatively small number of patients included in the study.

In conclusion, this study highlights the differences in chemotherapy regimen selection between elderly and non-elderly patients with advanced lung cancer. However, there were no significant differences in survival outcomes based on renal function. Larger studies are needed to further understand the relationship between regimen selection and prognosis in elderly patients with impaired renal function.
