This study aimed to identify the selection criteria for the initiation of different first-line treatment options in patients with metastatic non-small cell lung cancer (NSCLC). The researchers conducted an observational study involving multiple hospitals in Sweden, documenting factors such as patient parameters, tumor characteristics, and treatment choices. A total of 367 cases were included in the study, and the distribution of treatment options was as follows: chemotherapy immunotherapy (36.5%), chemotherapy (19.1%), best supportive care or palliative radiotherapy (28.1%), tyrosine kinase inhibitors (13.4%), immunotherapy (monotherapy) (14.2%), and other treatments (7.1%). The main factor influencing treatment choice was the Swedish National Guidelines in Lung Cancer (NVP) and multidisciplinary conferences, with 64% and 88.3% reliance, respectively. The most dominant factors in treatment selection were the patient's ECOG Performance Status (89.1%) and age (68.1%). The study concluded that immunotherapy in combination with chemotherapy was the most prevalent treatment option, and the decision-making process was largely guided by the national guidelines or multidisciplinary conferences. The study also highlighted the significant impact of ECOG performance status and age on treatment decisions. The findings provide important insights into the current clinical practice in Sweden and may have implications for other countries with similar treatment access limitations.
