This study aims to evaluate the accuracy of using simple multiples of the median overall survival (mOS) to estimate survival scenarios in randomized clinical trials (RCT) of immune checkpoint inhibitors (ICI) in advanced non-small-cell lung cancer (NSCLC). The researchers identified 15 trials with a total of 13,184 participants in 36 treatment groups. These treatment groups included a combination of chemotherapy and ICI in 9 trials, ICI alone in 6 trials, and chemotherapy alone in all 15 trials.<br /><br />The results showed that in groups treated with chemo-ICI, the simple multiples of the mOS accurately estimated the worst-case (90th percentile) and lower-typical (75th percentile) scenarios in 6 out of 9 groups. However, in the ICI-alone groups, these multiples were only accurate in 1 out of 9 groups for the worst-case scenario and 3 out of 9 groups for the lower-typical scenario. This discrepancy was due to the underestimation of early deaths in the ICI-only groups.<br /><br />The study also found that the 90th percentile (worst-case scenario) for ICI alone could be better estimated as 0.1 times the mOS, and the 75th percentile (lower-typical scenario) as 0.3 times the mOS. However, the 25th and 10th percentiles (longer term outcomes) could not be accurately estimated for most ICI-containing groups due to the short follow-up duration.<br /><br />In conclusion, simple multiples of the mOS accurately estimated the survival scenarios in the chemo-ICI groups but were inaccurate in the ICI-only groups. The researchers emphasize the need for longer follow-up in order to accurately estimate survival times beyond the mOS in trials of ICI.
