Pdf Summary
A quality improvement initiative was conducted in three community hospital-based cancer centers to improve the care of patients with advanced ALK non-small cell lung cancer (NSCLC) through a combination of certified education and quality improvement methods. The project involved baseline assessments of biomarker testing, diagnosis, and treatment selection for patients with advanced ALK NSCLC, followed by the development and implementation of improvement projects focused on biomarker testing and shared decision-making (SDM). Clinicians also participated in a live virtual educational intervention to learn about incorporating clinical advances and SDM strategies into treatment planning.

The results of the initiative showed improvements in biomarker testing and appropriate treatment selection for patients with ALK NSCLC. The use of ALK inhibitors in the first-line treatment of ALK NSCLC increased by 22%, and 92% of patients received molecular testing to guide treatment selection. The initiative also increased the practice skills necessary for integrating multigene panel testing, knowledge about the efficacy of ALK inhibitors, and the selection of appropriate first-line treatment of ALK NSCLC. The learners reported an increase in their intention to change practice, focusing on effective communication and care coordination.

However, the project faced challenges in coordinating testing with multiple reference laboratories, and future projects may help streamline the selection of laboratories for biomarker testing integration.

Overall, the initiative demonstrated the value and benefit of combining certified education and quality improvement methods in improving real-world community-based clinical practice for advanced ALK NSCLC. The results support the importance of shared decision-making and personalized treatment plans based on patient values and preferences.
