EP13.01. Does Oligometastases state exist in patients of Small Cell Lung Cancer? - A Small Retrospective Analysis from a Tertiary Cancer Centre - PDF(Abstract)

This study conducted at a tertiary cancer center aimed to evaluate the existence of an oligometastatic state in small cell lung cancer (SCLC) and the impact of its treatment on survival outcomes. Oligometastases refers to a limited number of metastatic sites. The concept of oligometastases is well established in other types of cancers, but not in SCLC due to its aggressive behavior.<br /><br />The retrospective study included SCLC patients with 1-3 sites of metastases. All patients underwent diagnostic PET-CT and MRI before treatment and received palliative chemotherapy. The study analyzed overall survival (OS) and progression-free survival (PFS) from the date of diagnosis.<br /><br />The results showed that out of 21 patients with oligometastatic SCLC, the most common metastatic sites were the brain, followed by the bone and other sites. The median number of chemotherapy cycles delivered was six. Some patients also received radiotherapy to the metastatic sites or the primary site.<br /><br />The median PFS and OS were 8 and 14 months, respectively. At 1 year, 70% of patients were alive. Radiotherapy to the primary site was found to be a significant prognostic factor for OS. Additionally, 5 out of 7 patients who developed progressive disease had recurrence at pre-existing oligometastatic sites.<br /><br />The study concluded that this small retrospective analysis showed improved survival outcomes in patients with 1-3 sites of oligometastatic SCLC compared to extensive-stage SCLC. However, further validation is needed in a larger prospective cohort. The addition of immunotherapy to intensify local therapy may also improve survival outcomes in carefully selected patients.<br /><br />In summary, this study suggests that some patients with small cell lung cancer may benefit from treating oligometastatic disease, but more research is needed to confirm these findings.
