Pdf Summary
This study aimed to examine the prognostic impact of the neutrophil-to-lymphocyte ratio (NLR) in patients with extensive-stage small cell lung cancer (ES-SCLC) receiving immune checkpoint inhibitors (ICIs) as treatment. The researchers analyzed data from advanced ES-SCLC patients who received first-line chemotherapy at their institution between January 2008 and July 2023. NLR measurements were taken at the initiation of each-line chemotherapy and at the end of treatment or death. The patients were divided into subgroups based on their NLR values and their time to treatment failure (TTF) and overall survival (OS) were compared.

The results showed that the change in NLR during treatment could be a useful biomarker for TTF and OS in ES-SCLC patients. The NLR at the start of first-line chemotherapy was also found to be correlated with the efficacy of ICIs. Additionally, multivariate analysis revealed that age, Eastern Cooperative Group performance status, and NLR were significant factors for OS.

Furthermore, the study investigated the effectiveness of immunotherapy in patients with high and low NLR values. The results showed a correlation between the NLR and overall survival in the NLR1-low subgroup, indicating that immunotherapy may be more effective in this population.

In conclusion, the NLR and its change during treatment could serve as prognostic predictors for patients with ES-SCLC. The NLR at the start of first-line chemotherapy may also help determine the need for additional treatment with ICIs. These findings highlight the potential of NLR as a biomarker for guiding therapeutic decisions and improving outcomes in ES-SCLC patients.
