EP13.01. Prognostic Nutritional Index-Based immune Risk System for Prognosis and Immunotherapy Response Prediction in Small Cell Lung Cancer - PDF(Abstract)

This study aimed to evaluate the prognostic role of the systemic tumor immune environment (STIE) in small cell lung cancer (SCLC) and identify potential beneficiaries of immune checkpoint blockade (ICB) therapy. The researchers enrolled 191 patients with stage I-III SCLC and used several quantitative measurements to assess the STIE. They also established a local immune score system (LISS) using machine learning algorithms. The value of the STIE was further tested in an independent cohort of advanced SCLC patients receiving PD-L1 blockade therapy.<br /><br />The results showed that the prognostic nutritional index (PNI), a measure of the STIE, was correlated with disease-free survival (DFS) and overall survival (OS) in SCLC patients. It remained as an independent prognostic factor for SCLC patients. Higher PNI scores were associated with an inflamed SCLC molecular subtype and greater local tumor-infiltrating lymphocytes. The researchers then constructed the LISS, which combined three important local immune biomarkers and integrated them with the PNI score. The integrated immune risk system was an independent prognostic factor and performed better than TNM stages and single immune biomarkers in predicting outcomes.<br /><br />In addition, SCLC patients with high PNI scores had better clinical responses and longer progression-free survival from PD-L1 blockade therapy. This study highlights the clinical application of the PNI in predicting efficacy and selecting beneficiaries for SCLC immunotherapy.<br /><br />Overall, this study provides a method to evaluate the prognostic value of the overall immune status in SCLC by combining the PNI with local immune biomarkers. It suggests that the PNI could be a useful tool in predicting clinical outcomes and identifying potential beneficiaries of immunotherapy in SCLC.
